Company Overview:

The Staten Island Workforce1 Career Center is partnering with the Community Agency for Senior Citizens (CASC) to find qualified candidates for a Community Outreach Specialist (Caregiver Program) in Staten Island, NY. CASC supports Staten Islanders age 60 and older in living independently by working collaboratively with older adults, respecting their independence, honoring their experiences, and strengthening community-based support systems.

Position Overview:
The Case Manager supports the Caregiver Services Program by conducting intakes, assessments, and service coordination for caregivers. This role helps connect clients to resources, develops support plans, and ensures services are delivered in accordance with NYC Aging standards.

This position is located in Staten Island, NY.
Key Responsibilities:
  • Conduct intakes, in-home assessments, reassessments, and case closures as needed
  • Develop and implement caregiver support plans, including coordinating respite and supplemental services
  • Connect caregivers to community resources and monitor service delivery
  • Provide guidance, advocacy, and follow-up support to caregivers
  • Maintain accurate and up-to-date case records and documentation
  • Build and maintain relationships with community partners and service providers
  • Ensure compliance with NYC Aging standards and program requirements
  • Assist with administrative tasks and additional program activities as needed
Requirements:
  • Bachelor’s degree in Social Work or a related field required
  • At least 1 year of experience in a social service setting preferred
  • Experience working with older adults and/or caregivers preferred
  • Strong communication, organizational, and problem-solving skills
  • Ability to work collaboratively and engage with diverse populations
  • Knowledge of community resources and service systems a plus
  • Proficiency in documentation and case management systems
  • Bilingual skills are a plus
Pay Range: USD $22.59 - USD $24.03 /Hr.
