Title: Case Manager
Schedule: Monday-Friday, 8:30 AM-5:00 PM
We are seeking compassionate and dedicated Case Managers to support individuals experiencing homelessness on their journey toward housing stability and self-sufficiency. This role provides direct client support, case management, service coordination, and resource navigation while working collaboratively with internal teams and community partners. The ideal candidate is passionate about helping vulnerable populations, skilled at building trusting relationships, and experienced in connecting clients with housing, employment, healthcare, and community resources.
Responsibilities
Conduct client assessments to identify immediate and long-term service needs.
Develop individualized service plans focused on housing stability, employment, income growth, healthcare access, and personal goals.
Provide ongoing case management, client advocacy, and regular follow-up meetings to monitor progress.
Assist clients with obtaining identification, public benefits, healthcare services, and other supportive services.
Connect clients with employment opportunities, job training programs, education resources, and community-based supports.
Maintain a trauma-informed, person-centered, and culturally responsive approach in all client interactions.
Collaborate closely with shelter staff, housing navigators, engagement specialists, and community partners to coordinate client care.
Provide services throughout shelter, day center, hygiene center, and campus-based program locations.
Facilitate referrals to housing resources, behavioral health services, workforce development programs, legal aid organizations, and healthcare providers.
Advocate for clients and assist them in navigating complex systems and service networks.
Participate in multidisciplinary team meetings, case conferences, and coordination efforts to support successful client outcomes.
Maintain accurate and timely client records, case notes, and service documentation.
Enter and manage data within HMIS and other case management systems.
Ensure compliance with agency policies, confidentiality guidelines, and program requirements.
Track client progress, outcomes, and service engagement activities.
Prepare reports and documentation supporting program performance and funding requirements.
Qualifications
Associate's or Bachelor's degree in Social Work, Human Services, Psychology, or a related field preferred.
Minimum of 1-2 years of case management, homeless services, social services, housing programs, or related experience.
Experience working with diverse and vulnerable populations.
Knowledge of community resources, public benefits, housing programs, and employment services.
Strong interpersonal, communication, crisis intervention, and problem-solving skills.
Ability to manage multiple priorities while maintaining accurate documentation and meeting deadlines.
Experience with Microsoft Office Suite and data entry systems; HMIS experience preferred.
Bilingual English/Spanish skills are highly preferred.

Pay Details: $25.00 to $30.00 per hour
