Who You Are:

Amoskeag Health is seeking a dedicated Substance Use Disorder (SUD) Case Manager to work closely with our Medication Assisted Treatment (MAT) team in supporting patients on their recovery journey from substance use disorders. You are a compassionate, patient-centered professional who thrives in a collaborative, integrated care environment. You excel at building relationships with patients, medical providers, behavioral health clinicians, and community partners to coordinate care and improve health outcomes. You are committed to providing culturally competent services to diverse populations and are adaptable, resourceful, and able to effectively manage competing priorities in a fast-paced environment while supporting patients through every stage of recovery.

What You'll Do:

  • Provide individualized case management support to meet each patient's needs and updates their treatment plan regularly.
  • Responsible for patient data collection and reporting to support grant funding.
  • Work with integrated MAT team to assist with addressing vulnerabilities in social determinants of health.
  • Work with MAT team, supervisor, and interpreters to identify language and low-literacy materials to use with patients to provide information and resources to better manage their health condition(s).
  • Assess potential resources to provide education to patients such as: Pamphlets, videos, posters, classes, one on one education and share with MAT Team for consensus and planning.
  • Work directly with patients to ensure coordination of healthcare and community service support providers to reduce barriers in accessing needed services.
  • Document interventions directly in patient's EMR (Electronic Medical Record). Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place.
  • Facilitate communication and coordinate services between providers and community agencies.
  • Responsible for care coordination of new patients interested in the MAT program.
  • Complete brief SUD screening to aid in determining appropriate level of care and motivation.
  • Utilize CRSW skills and training to facilitate individualized patient recovery journey.
  • Lead and / or assist with facilitation of recovery groups both virtual and in-person
  • Attend SUD related meetings in the community to promote MAT program, stay connected to SUD developments in the area, make and maintain SUD organization relationships.
  • Establish positive, supportive relationships with patients, families, and visitors.
  • Motivate patients to be active, engaged participants in their mental and physical health.
  • Effectively work with people (staff, clients, doctors, agencies, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions.
  • Build and maintain positive working relationships within the integrated team of providers, nurses, case managers, agency representatives, medical assistants and supervisor.

What You'll Bring:

  • Bachelor's degree in healthcare administration or a related field; or an equivalent combination of education, training, and relevant experience.
  • Certified Recovery Support Worker certificate (CRSW) required or working towards obtaining.
  • Any combination of 3 years health care/substance use disorder services experience.
  • Verifiable good driving record and reliable transportation
  • Demonstrated ability to manage multiple priorities, collaborate with cross-functional teams, and provide effective support in a fast-paced healthcare environment.
