Case Manager - Veteran Care Coordinator - Boise 23rd

GENERAL RESPONSIBILITIES

Coordinates and provides supportive services including but not limited to suicide screenings, outreach, social service referrals, crisis intervention, case management, client advocacy and related services to individuals and families.

MINIMUM QUALIFICATIONS

The following minimum qualifications are required for this position. All required education, licensure, certifications, registrations, and other job specific credentials must be obtained and successfully verified prior to the employee's start date.

• Bachelor's degree or undergraduate degree OR four (4) years of equivalent experience.
• Two (2) years of experience in case management, care coordination, behavioral health, social services, healthcare, or a related field.
• Experience working with veterans and military-connected individuals.
• Licensed, or license-eligible, in the State of Idaho (if relevant for degree).
• Demonstrated willingness and desire to work with the veteran population.
• Demonstrated understanding of the problems for those patients with all manner of behavioral health issues and difficult patient populations.
• A working knowledge of service area social service resources for those patients with behavioral health issues (city, county, state and federal).
• Ability to work independently.
• Excellent communication and customer service skills.
• Experience with grant-funded programs and data collections.

PREFERRED QUALIFICATIONS

• Veteran or military experience.
• Experience working with complex patient populations.
• Experience working in a healthcare setting.
• Experience working in a multidisciplinary setting.
