 Alfred J. Thomas Home for Veterans

 SHIFT: Monday-Friday: (Monday & Friday: 8:00am-4pm / Tuesday-Thursday: 2pm-10pm)

 Provides case management and support services to veterans enrolled in a residential veterans’ program. 

  •  Provide on-site support and supervision to veterans in a residential setting.
  • Conduct assessments, develop recovery plans, coordinate care, and connect veterans with community resources and supportive services.
  • Facilitate individual support, group sessions, family meetings, case conferences, and discharge planning.
  • Advocate for veterans and promote recovery, independence, and overall well-being.
  • Lead recreational, socialization, and recovery-focused activities that support veteran engagement and growth.
  • Monitor and document services provided while maintaining accurate, timely, and confidential records.
  • Observe self-administration of prescribed medications and provide medication education in accordance with agency procedures.
  • Ensure a safe, supportive, and well-maintained environment by following fire and life safety procedures and reporting maintenance needs.
  • Safely transport veterans to appointments and community activities using agency vehicles when needed.
  • Collaborate with team members to support daily program operations, maintain clear communication, assist with shift coverage, and participate in trainings and staff meetings.
  • Support agency billing, documentation, and compliance procedures with accuracy and consistency.

 Qualifications:

  • Bachelor’s Degree in Social Work, Psychology or a related field.
  • One year of mental health, or substance abuse, or veteran services experience.
  • Must possess a valid driver’s license, with a good driving record.
  • Military or Veteran work history preferred.
