Case Manager

Registered Nurse Case Manager – Direct Hire

Location: Wheeling, IL Schedule: Monday–Friday, 8:00 AM–5:00 PM (daily start/end times may vary) On‑Call Requirements:

• One (1) 24‑hour weekend day shift per month
• Two (2) weekday on‑call shifts per month (5 PM–8 AM)
• Two (2) holidays per year (1 summer, 1 winter)
• Three (3) floating holidays
• Additional stipends and compensation provided for on‑call coverage
• Dedicated after‑hours RN team (3 RNs nightly)

Overview

This is an excellent opportunity for a compassionate and experienced Registered Nurse Case Manager to join a supportive hospice team in Wheeling, IL. In this role, you will provide essential care coordination, leadership, and patient advocacy for individuals receiving hospice services. You'll work closely with interdisciplinary teams to ensure patients and families receive exceptional end‑of‑life care.

Key Responsibilities

• Manage and oversee patient care within hospice settings
• Conduct comprehensive assessments and develop individualized care plans
• Coordinate care with physicians, social workers, and interdisciplinary teams
• Provide leadership, mentorship, and support to nursing staff
• Ensure compliance with hospice standards, policies, and regulatory requirements
• Maintain accurate documentation and timely reporting
• Travel locally to visit patients across multiple counties

Required Qualifications

Education:

• High School Diploma or GED

Experience:

• 1+ year Hospice experience
• 1+ year Management or Leadership experience
• 1+ year experience in ANY of the following:
  • Home Health Care
  • Epic Home Health
  • Home Health

Licensure:

• Valid Registered Nurse (RN) license in the state of Illinois (IL)

Certifications (ALL required):

• American Heart Association Basic Life Support (AHA BLS)
• American Heart Association Advanced Cardiac Life Support (AHA ACLS)

Schedule & Shift Details

• Monday–Friday, 8:00 AM–5:00 PM (flexible daily start/end times)
• On‑Call:
  • One 24‑hour weekend day per month
  • Two weekday on‑call shifts (5 PM–8 AM)
  • Two holidays per year (1 summer, 1 winter)
  • Three floating holidays
• Dedicated after‑hours RN team supports overnight coverage

Travel Requirements

• Local travel required across multiple counties
• Must be comfortable working in field‑based hospice environments
