The Staten Island Workforce1 Career Center is partnering with Seamen’s Society for Children and Families to find qualified candidates for a Case Planner – Family Support Services in Staten Island, New York. Seamen’s Society for Children and Families is a nonprofit organization that has supported children and families for more than 175 years. The organization provides programs that help vulnerable children and families achieve greater safety, stability, and well-being through compassionate services, community support, and dedicated professionals working to create brighter futures.

Position Overview:

The Case Planner – Family Support Services provides direct services to children, individuals, and families experiencing complex trauma, abuse or neglect, and intimate partner violence. The position conducts assessments, develops service plans, provides crisis intervention and counseling, and connects families with community-based resources. The Case Planner works closely with families, community partners, and the Administration for Children's Services (ACS) to support family stabilization, safety, and well-being while maintaining timely documentation and program records.

 

This position is located in Staten Island, NY.

Key Responsibilities:
  • Assess children, individuals, and families and connect them with community resources and supportive services.
  • Provide case planning, assessments, crisis intervention, counseling, and service planning.
  • Conduct family assessments and complete required reviews within program timelines.
  • Collaborate with ACS, legal teams, case aides, and community partners to support family service needs.
  • Maintain case records, tracking logs, and required program documentation in a timely manner.
  • Participate in required meetings, conferences, supervision, and agency training.
  • Monitor children's educational progress and families' medical needs.
  • Develop service plans that promote safety and stabilization using a harm reduction approach.
Requirements:
  • Bachelor's degree. required
  • At least two years of child welfare and counseling experience.
  • Bilingual in Spanish, Chinese/Mandarin, Russian, Italian, or French Creole preferred.
  • Experience using Microsoft Word, Excel, PowerPoint, CNNX, and PROMIS.
  • Ability to communicate effectively with families, supervisors, coworkers, and community partners.
  • Ability to organize work, manage priorities, and respond to changing program needs.
  • Demonstrated collaboration, facilitation, and cultural competency skills.
  • Work hours are based on program needs and may vary.
Pay Range: USD $26.67 - USD $26.67 /Hr.
