POSITION SUMMARY

The Case Worker -Service Facilitator works with assigned participants who are diagnosed with mental health and/or substance use diagnoses throughout the lifespan. This position creates a person-centered, multi-disciplinary team which promotes participant achievement for their highest possible level of independent functioning within their community

QUALIFICATIONS

  • Serves as member of a multi-disciplinary team, supporting individuals with a recoverable mental health disorder(s) and/or a substance use disorder(s) utilizing principles of psychosocial rehabilitation; adheres to State Statutes, Administrative Code, County and Department of Health and Human Services policies/procedures and social work practice standards.
  • Identifies full range of client needs through thorough investigation and assessment; differentiates between medical, psychological, vocational, social, familial, and daily living spheres; works to achieve voluntary involvement and participation in the design of a self-directed recovery plan developed to support the participant, in a manner that helps achieve the highest possible level of independent functioning.
  • Provides service facilitation and coordination for participants across the life span; gathers, coordinates, and schedules regular participant recovery-based team meetings; transfers information, reviews progress, identifies barriers or provides support to the assigned participant's individualized needs; ensures that the recovery plan and services delivered are integrated, coordinated, and monitored.
  • Extends support for individuals, families, or significant others in individual, conjoint, and group formats; promotes and teaches independence, resiliency, connections to community, and informal supports to enhance and sustain the recovery process.
  • Adheres to strict record-keeping and reporting guidelines and laws in compliance with agency-defined fiscal parameters and DHS 36 required timelines; documents client contacts and services; maintains client files and client specific data for reporting purposes; creates authorizations of services for payment per an individual's recovery plan; prepares other records and reports, as necessary.
  • Maintains awareness of community resources and support services available for individuals in crisis; educates clients about available community resources and how to independently gain access to needed services.
  • Establishes and maintains effective working relationships with internal and external partners; serves as advocate for clients within the community; attends and participates in staff meetings, supervision times, and other regularly scheduled meetings.
  • Develops professional knowledge and skills by attending training events, conferences, and workshops.
  • Performs other related duties as required or assigned.

TRAINING AND PROFESSIONAL EXPERIENCE QUALIFICATIONS

  • Bachelors degree in social work or related field.
  • Appointment will be conditional upon successful completion of criminal and caregiver background checks.
  • Must have valid driver's license, an acceptable driving record, and access to private transportation for work-related duties.
  • Must possess and maintain personal automobile insurance in the amount of $100,000/$300,000.

DESIRED TRAINING AND PROFESSIONAL EXPERIENCE QUALIFICATIONS

  • Certified Social Worker (CSW), Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), Licensed Clinical Social Worker (LCSW), or Licensed Professional Counselor (LPC).

Paid Time Off (PTO): Accrual is based on years of service with the County, starting at 6.4 hours accruing each pay period (20.8 days per year), with the maximum accrual rate of 35.75 days per year. All new hires at Eau Claire County receive 40 hours of front-loaded PTO upon hire, which they may begin using immediately.

Holidays: Eleven paid holidays: New Years Day, Martin Luther King, Jr. Day, a Spring Holiday, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, the Day after Thanksgiving, Christmas Eve Day, Christmas Day and New Years Eve Day.

Retirement: The County provides a retirement account in Wisconsin Retirement System (WRS). Employees pay one-half of the actuarially required contributions.
