Case Worker
Location: New York, NY
Type: Contract (8 Weeks)
Pay: Local: $36.00 – $41.00/hr | Traveler: $45.00 – $50.00/hr
Shift & Schedule: Monday – Friday | 9:00 AM – 5:00 PM
We are seeking an experienced Case Worker to provide social service support, care coordination, and resource management services. This role focuses on assessing client needs, coordinating community resources, supporting public assistance programs, and maintaining accurate case documentation. The ideal candidate will possess strong organizational, communication, and case management skills with experience working in healthcare or social service environments.
• Conduct assessments to identify social service and resource needs.
• Develop, implement, and monitor individualized service plans.
• Assist clients with public assistance, food stamps, medical benefits, and community resources.
• Evaluate eligibility for social service programs and support services.
• Provide guidance, counseling, and referrals to appropriate resources.
• Coordinate services with healthcare providers, agencies, and community organizations.
• Maintain accurate case records, documentation, and reports.
• Perform data entry and case tracking within electronic systems.
• Utilize EPIC and CarePort systems for care coordination and documentation.
• Investigate and assess social service concerns and client needs.
• Assist individuals requiring institutional care or protective services.
• Monitor case progress and ensure timely follow-up activities.
• Maintain confidentiality and comply with organizational policies and procedures.
• Bachelor's Degree from an accredited college or university.
• Minimum 3 years of case management, social services, or related experience.
• Experience working with public assistance, healthcare, or community service programs.
• EPIC experience required or highly preferred.
• Strong documentation, data entry, and case tracking skills.
• Experience coordinating services for diverse populations.
• Ability to work onsite Monday through Friday.
• Degree in Social Work, Human Services, Psychology, Sociology, or related field.
• CarePort experience.
• Social service or case management certifications.
• Experience in hospital, community health, or public assistance settings.
