SUPERVISORY RESPONSIBILITIES:

On-site primary direct supervision of all staff in supervisory functions of those programs under his/her responsibility. Demonstrates the ability to direct and motivate staff for both CAT programs.

 

JOB SUMMARY:

The Community Action (CAT) Program Manager oversees patient services provided by the CAT programs. This position is responsible for providing administrative and clinical supervision to all staff assigned to his/her program areas of responsibility.

Adheres to meeting program standards, staffing, expectations, and performances targets as delineated by LSF, the managing entity, including required reports.

Demonstrates an excellent working knowledge of each specific program's directives and goals

Displays strong working knowledge of healthcare standards

Develops sound working relationships and subordinates to enhance the likelihood of effective outcomes for Center program and clients

Develops sound working relationships with personnel of community organizations to impact successful outcomes of the Center mission and vision

Manages and oversees the day-to-day operations of patient services provided within the continuity of care continuum

Monitors CAT programs for clinical and fiscal effectiveness

Evaluates effectiveness and promotes opportunities for improvement

Directs scope of patient services needed through discussion with the leadership and Management Team

Conceptualizes service delivery for those programs under his/her direct supervision, establishes goals and engages in improving the programs' outcomes and effectiveness in line with services necessity and fiscal concerns.

Participation in on-call rotation for CAT Team.

Conduct youth screenings for counties served by CAT at Lifestream: Citrus, Hernando, Lake and Sumter

JUDGMENT/DECISION MAKING:

EDUCATION & EXPERIENCE:

Education:

  • Master's degree required in behavioral healthcare field. Licensed as LCSW, LMHC and/or LMFT required. Qualified Supervisor preferred.

Experience:

  • Minimum of two (2) years' experience as a direct care provider for youth & families required. Wrap-around training coaching preferred or to be completed within 90 days of hire. Experience in Baker Acts preferred.

Driving Requirements:

  • Must be Over the age of 21 required
  • Minimum of 5-years driving experience required
  • Possess a Valid FL Driver's license (within 30 days of hire if currently do not possess)
  • Possess a continuously valid US Driver's license for the past three years, from date of issue required
  • Safe Driving record required

 

TRAINING & DEVELOPMENT:

Completes all requirements according to Individual Training Plan within the first six months of hire and annually. Other training and/or professional development may be assigned due to evolution of programs.
