Job Summary and Responsibilities
As our Licensed Clinical Social Worker, you will coordinate and implement post-discharge plans in close coordination with Case Managers, utilizing the Extended Care Information Network (ECIN).
Every day you will manage patient discharge processes, provide essential advocacy for patients, and facilitate referrals to critical community resources, ensuring seamless transitions of care and comprehensive support.
To be successful in this role, you will be a Licensed Clinical Social Worker with expertise in discharge planning and community resource navigation. Your strong organizational skills, ability to utilize ECIN, and commitment to patient advocacy are essential for effective care coordination.
This is an exciting opportunity for a behavioral health clinician who is passionate about meeting patients where they are and providing meaningful, patient-centered mental health care throughout their hospital journey. This position serves as a therapist within the hospital, supporting patients from the moment a behavioral health need is identified through crisis intervention, treatment, and inpatient care. Clinicians will conduct level-of-care assessments, provide crisis intervention and disposition planning, and follow an inpatient therapy caseload, allowing them to work face-to-face with patients throughout different stages of their behavioral health journey.
The role includes facilitating therapeutic groups, providing individual support and interventions, and working closely with providers, nursing, and the interdisciplinary care team to ensure each patient's mental health needs are heard, understood, and addressed. Master's Degree Soical work
Licensed Clinical Social Worker
Knowledge of community resources used for discharge planning, hospital operations, excellent communication/presentation skills
Knowledge of third party payment systems, Medicare/Medicaid programs
Maintains current knowledge base of community services through continuing education
