SUMMARY: Responsible to ensure that competent, safe quality and professional care is provided to each of the patients assigned to his/her caseload or other team members (in their absence), in accordance with current Federal, State and local standards, guidelines and regulations. Utilize knowledge of chemical dependency and counseling techniques to introduce effective education and counseling for patients under his/her care and in each unit under other clinical staff.
ESSENTIAL DUTIES AND RESPONSIBILITIES: Includes the following. Other duties may be assigned.
Provides quality care to the assigned patients, in terms of compliance with facility expectations, SMA and other licensing boards assuring effectiveness of the therapeutic services.
Conducts individual and group counseling to meet treatment plan goals and objectives.
Produce a clear and concise documentation in all services including, but not limited to: Admit note, assessment, daily charting, weekly clinical review, staffing notes, treatment plans updates, biopsychosocial, transition and discharge planning, documentation of family sessions as needed and all summaries.
Responsible for appropriate referral to each patient in need, open clear and ethical communication with referral sources: follow-up of each of his/her cases after discharge.
Appropriately and professionally provides case management, patient education, and keep clear, ethical and confidential reports and record keeping. Interacts and relates with peers and other professionals to assure comprehensive and quality of care for the patients.
SUPERVISORY RESPONSIBLITIES: None. Reports to the Program Director and work closely with all clinical team.
QUALIFICATION REQUIREMENTS:
MUST BE A CADC or CADCI to qualify.
Bachelor's Degree in Psychology, Counseling, Social Work, or related field or Associate's Degree in Counseling or related field. Registration with the North Carolina.
Substance Abuse Board is required as well as supervision if you are not a licensed or certified substance abuse counselor. Experience in the field may offset formal education requirement. If in recovery, at least 2 years continuous sobriety required.
Ability to work independently as well as with the team, ability to maintain high level of professional ethics.
LANGUAGE SKILLS:
Ability to ready, analyze, and interpret general clinical reports and documents.
Ability to respond to regulatory agencies. Ability to effectively present information to supervisor.
CERTIFICATES, LICENSES, REGISTRATIONS:
LCDC, HIV/AIDS Education and non-violent crisis intervention.
