Provides state of the art diabetes education in the outpatient setting to patients with diabetes and their families/care partners in order to optimize diabetes management. Participates in the development of educational materials and programs and teaches both individuals and groups based on practice guidelines. Uses creative teaching strategies to promote lifestyle changes that improve outcomes. Collaborates with other members of the team in teaching well day and sick day adjustments, home monitoring and exercise. Uses principles of growth and development to assess each patient's age specific needs, treatment and care.

EDUCATION
RN required, BSN/MS preferred
EXPERIENCE
Preference given to those with at least two years of demonstrated experience and expertise in diabetes education. Exceptional skills in both verbal and written communication. Comfort with public speaking. Ability to work in a multidisciplinary setting.
LICENSURE
Must have a current State of Connecticut nursing license.
SPECIAL SKILLS
Certified by the National Certification Board for Diabetes Educators (CDE) or eligible to sit for examination within 12 months.
