Description
Grace Healthcare Services is hiring Certified Home Health Aide Liaison to cover Southern, NJ. Must have flexibility. This is a field-based position. Have realaible transportation
PURPOSE: The Home Health Aide Liaison is an experienced and skilled individual whose primary responsibilities are to develop, mentor, and support HHAs in the field. The person in this position will assist RNs and Regional Managers to evaluate home health aides in the field to ensure all Grace HCS expectations are met and education is provided as needed.
QUALIFICATIONS:
High school graduate or equivalent
Current CHHA License is preferable
Three years related work experience
Excellent communication and interpersonal skills
Able to multitask efficiently
Excellent customer service skills
Proficient in Word, Excel, And Outlook
RESPONSIBILITIES (including, but not limited to):
This position has supervisory duties.
Supervise home health aides along with RNs.
Conduct in-field evaluations of home health aides and provide education when necessary.
Complete home health aide report and submit them to the HR department and appropriate parties.
Alert management when a home health aide is not properly trained or is not performing tasks correctly.
Provide employee counseling along with RN/HR when necessary to resolve issues.
Ensure that any issues involving facility staff or family members are reported to the RN, Regional Manager, and HR Department.
Act as a liaison between home health aides, the interdisciplinary team, and management.
Provide education during orientation and aide in-services.
Review care plans with agency home health aides, ensure correct assignments are done, and paperwork is completed accurately.
Adhere to the practice of confidentiality with regard to patients, staff, customers, and all aspects of the organization.
Ensure best practices of customer service and professionalism in all interactions with field staff, peers, supervisors, and customers.
Ensure that Grace Healthcare Services'' best practices are incorporated into all job activities/responsibilities.
