Belle, LLC is a healthcare company focused on improving outcomes for seniors through in-home preventive foot care, community health workers, and telehealth-supported care management. In this role, you’ll help deliver high-quality, member-centered care and play a key part in improving clinical outcomes, supporting care coordination, and enhancing the experience for both members and their caregivers. This opportunity is well suited to a compassionate medical assistant who is passionate about improving the health and well-being of older adults through personalized, holistic support.
Responsibilities
Review Community Health Worker (CHW) appointment documentation for accuracy and completeness, escalating issues to the nurse case manager as needed.
Support care coordination activities, including scheduling follow-up appointments and communicating with members or their caregivers.
Mail and/or email patient care plans and educational materials as directed by case managers.
Accurately update and maintain patient records within the electronic health record (EHR) system.
Protect patient confidentiality and support compliance with HIPAA and organizational policies.
Requirements
Experience, Skills, & Personality
Certified or Registered Medical Assistant (CMA/RMA) required.
2 years of experience in a medical office, telehealth, or care coordination setting.
Experience using EHR systems; experience using eClinicalWorks preferred.
Experience working within HIPAA and organizational policy requirements in a healthcare setting.
Available during Eastern Time business hours.
Experience working with older adult populations preferred.
