Position Overview
The Home Health Aide (HHA) / Certified Nursing Assistant (CNA) is a paraprofessional member of the home care team who provides personal care services to patients under the supervision of a Registered Nurse or therapist.
In this role, you will support patients with daily activities, monitor their condition, and communicate observations to the clinical team to ensure safe, high-quality care.
Responsibilities
Provide personal care assistance such as bathing, grooming, dressing, and mobility support
Observe and report changes in patients’ condition to the supervising nurse or therapist
Document care provided and patient observations accurately
Follow the care plan established by the Registered Nurse
Maintain a safe and supportive environment for patients in their homes
Promote quality, continuity, and safety of patient care
Qualifications
Certified Nursing Assistant (CNA) in Massachusetts preferred; Home Health Aide certification accepted
Must be 18 years of age or older
Ability to read and follow written care plans and document services provided
Ability to work independently with minimal supervision
Compassion and empathy for individuals who are ill, injured, or elderly
Current CPR certification required
Strong interpersonal skills, patience, and professionalism
Valid driver’s license and reliable insured vehicle
Ability to assist patients with repositioning, transferring, and other physical tasks.
