Job Responsibilities:<\/span><\/b>
<\/div>Provide hands -on personal care assistance as outlined in the care plan<\/span>
<\/div>
Assist with bathing, grooming, dressing, and other activities of daily living<\/span>
<\/div>
Provide mobility assistance and help maintain a safe home environment<\/span>
<\/div>
Assist with light housekeeping and homemaker duties<\/span>
<\/div>
Help with meal preparation and basic household support as needed<\/span>
<\/div>
Provide companionship and emotional support<\/span>
<\/div>
Observe and report any changes in the client’s condition or needs<\/span>
<\/div>
Maintain professionalism and respect client privacy and dignity<\/span>
<\/div>
Follow Matrix Home Care policies and the client’s individualized care plan<\/span>

Communicate clearly with the care team regarding any concerns or updates<\/span>
