Description
Job Summary Ascend Home Health & Hospice is seeking a compassionate and dedicated Certified Nursing Assistant to join our Ascend Health team. The ideal candidate will provide essential support to patients in various settings, including nursing homes, assisted living facilities, and home care environments. This role requires a strong commitment to patient care, with responsibilities that include assisting with daily living activities, monitoring vital signs, and ensuring the comfort and safety of patients.
Responsibilities
Assist patients with daily living activities such as bathing, dressing, grooming, and feeding.
Monitor and record vital signs including temperature, pulse, respiration, and blood pressure.
Provide support in mobility and transportation of patients using equipment such as Hoyer lifts.
Maintain a clean and safe environment for patients by adhering to infection control protocols.
Document patient care activities accurately in electronic health record (EHR) systems.
Communicate effectively with healthcare team members regarding patient status and needs.
Assist in administering medications under the supervision of licensed nursing staff.
Support patients with special needs including those with developmental disabilities, Alzheimer's disease, dementia care, or other cognitive impairments.
Participate in patient care planning and contribute to the overall well-being of residents or patients in long-term care settings.
Requirements
Current Certified Nursing Assistant (CNA) certification
Home Health and/or Hospice experience is preferred.
Knowledge of medical terminology, anatomy, physiology, and HIPAA regulations is essential.
Ability to work effectively with diverse populations including geriatrics.
Strong interpersonal skills for working closely with patients and their families.
Basic life support (BLS) certification is required; additional certifications in specialized areas (e.g., hospice care, behavioral health) are beneficial.
