Certified Nursing Assistant (CNA). Flexible Schedule, Compassionate Care! As a CNA at PASCO/SW Home Health & Hospice, you’ll provide essential hands-on support to patients and families, ensuring comfort, dignity, and quality of life. You’ll assist with daily living activities, collaborate with a caring interdisciplinary team, and make a meaningful impact, all while enjoying the flexibility you need for a healthy work-life balance.

Key Responsibilities
• Provide personal care, including bathing, grooming, oral hygiene, shampoos, and changing bed linens
• Assist with dressing, toileting, and mobility needs
• Plan and prepare nutritious meals; assist with feeding as needed
• Take and record vital signs (temperature, pulse, respiration, blood pressure) when ordered
• Perform range of motion exercises and simple therapy procedures as instructed
• Monitor and report changes in patient condition to the supervising nurse
• Maintain a clean, safe, and comfortable environment for patients
• Offer emotional support and companionship to patients and families
• Provide respite care for family/caregivers when appropriate
• Participate in care planning, team meetings, and quality improvement activities
• Adhere to documentation standards and professional conduct guidelines
Requirements
• Current CNA certification and successful completion of state-approved training and competency evaluation
• At least 18 years of age
• Ability to read and follow written instructions and document the care provided
• Comfortable providing specialized care to terminally ill patients, understands hospice philosophy
• Empathy, tact, patience, and strong communication skills
• Current CPR certification
• Valid driver’s license, automobile insurance, and reliable transportation for agency-related travel
• Good organizational skills and ability to work independently

Pasco/SW Home Health was founded by Yvette Tanner in 1999 as a sister agency to PASCO in Denver. Our passion for home health care started with caring for our very first patient, Yvette’s son. PASCO/SW later split into an independent agency in 2006 and in 2021, became part of the Cornerstone Inc. family.
