Role Overview The Certified Nursing Assistant (CNA) delivers compassionate, participant-centered personal care to frail seniors within our Adult Day Health Centers and in participants' home environments. Working directly alongside an Interdisciplinary Team (IDT), this role supports participants' daily physical, emotional, and social needs while following individualized care plans. Key Responsibilities
Activities of Daily Living (ADLs): Assist participants with bathing, dressing, grooming, toileting, dining, and safe mobility/transfers using proper body mechanics and adaptive equipment.
Health & Vital Monitoring: Measure, track, and record baseline vital signs (blood pressure, pulse, temperature, weight) and immediately report any changes in condition or behavior to the primary clinic nurse.
Program & Clinic Escort: Escort participants to onsite clinic visits, physical/occupational therapy appointments, and structured recreational activities.
In-Home Support (when assigned): Deliver one-on-one personal care, assistance with meals, and light homemaking in participant residences as specified by the home care plan.
Documentation: Accurately document daily care, observations, and vitals in the Electronic Health Records (EHR) system adhering to regulatory standards and facility policies.
Safety Protocols: Follow strict infection control practices, utilize standard precautions, and implement fall prevention measures to ensure a safe environment.
Qualifications
Certification: Active California Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification.
Life Support: Current Basic Life Support (BLS) / CPR certification (American Heart Association or Red Cross).
Experience: Minimum of 1 year of hands-on experience working with a frail or elderly population in a long-term care, home health, PACE, or adult day care setting.
Communication Skills: Strong verbal and written English skills; bilingual language ability is a plus depending on center demographics.
Physical Requirements: Ability to stand, bend, stoop, and lift/transfer up to 50 lbs safely using appropriate equipment and techniques.
