Day Center Personal Care Assistant / Caregiver

Location: San Jose, CA — On-site
Contract Length: 3 Months
Schedule: Monday–Friday, 8:00 AM–4:30 PM

We are seeking a compassionate and dependable Day Center Personal Care Assistant to support frail and elderly adults. In this role, you’ll provide hands-on care and work alongside our care, nutrition, and engagement teams to help participants enjoy a safe, welcoming day center experience.

What You’ll Do

• Assist with bathing, dressing, grooming, toileting, and meal assistance.

• Measure and document vital signs and report changes in participants’ physical or mental condition.

• Help with safe transfers, boarding and exiting transportation vehicles, and escorting participants to medical appointments.

• Support meal services according to individual dietary plans.

• Assist with activities that encourage engagement and social interaction.

• Maintain personal care supplies and keep participant areas clean and organized.

• Complete timely, accurate documentation in Electronic Medical Record (EMR) systems.

Required Qualifications

• Minimum one year of experience providing personal care to frail or elderly individuals.

• High school diploma/equivalent OR California DPH-issued CNA.

• Valid California Food Handler’s Certificate.

• Experience navigating and documenting in EMR systems.

• Strong communication, organization, and prioritization skills.

• Availability for occasional evenings, weekends, or overtime as needed.
