Job Summary
Nexus at Geneva is seeking a dedicated Per Diem Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team in a skilled nursing and post-acute care setting.
This role offers flexible, as-needed scheduling while supporting Occupational Therapists in delivering high-quality, patient-centered occupational therapy services. The COTA will assist in implementing treatment plans to help residents improve independence with activities of daily living (ADLs), functional abilities, and overall quality of life.
Key Responsibilities
- Implement occupational therapy treatment plans under the supervision of an Occupational Therapist
- Provide skilled interventions to improve ADLs including dressing, bathing, grooming, eating, and mobility
- Assist residents with therapeutic exercises, adaptive techniques, and use of assistive equipment
- Observe, record, and report resident progress and responses to treatment
- Collaborate with Occupational Therapists, Physical Therapists, Speech-Language Pathologists, nursing staff, and physicians
- Participate in interdisciplinary care planning and team meetings
- Document treatment sessions in compliance with facility and regulatory standards
- Educate residents and caregivers on home exercise programs and functional strategies
- Support safe and effective transitions of care and discharge planning

Qualifications
- Associate degree from an accredited Occupational Therapy Assistant program
- Active Illinois Certified Occupational Therapy Assistant (COTA) license
- NBCOT certification (as required for COTA credentialing)
- CPR certification (commonly required in SNF settings)
- Prior experience in skilled nursing, rehab, or acute care preferred
- Strong communication, documentation, and teamwork skills
