Full Job Description
Nexus at Belleville is seeking a Certified Occupational Therapy Assistant (COTA) to provide high-quality occupational therapy services in a skilled nursing setting. This role supports residents in improving independence, function, and quality of life through individualized treatment interventions under the supervision of a licensed Occupational Therapist (OT).

Responsibilities
- Implement occupational therapy treatment plans developed by the supervising Occupational Therapist
- Assist residents in improving activities of daily living (ADLs), including dressing, bathing, grooming, and mobility
- Provide therapeutic interventions to address cognitive, physical, and functional limitations
- Monitor patient progress and report responses to treatment to the supervising OT
- Assist with restorative and maintenance therapy programs as needed
- Document treatment sessions, progress notes, and resident responses accurately and timely
- Collaborate with an interdisciplinary care team including nursing, rehabilitation staff, and physicians
- Educate residents, families, and caregivers on adaptive techniques and safe functional practices
- Support discharge planning and transition of care when appropriate
Qualifications
- Associate’s degree from an accredited Occupational Therapy Assistant program
- Active Illinois COTA license (required)
- Current NBCOT certification (required)
- Experience in skilled nursing or long-term care preferred
- Strong understanding of ADL training, functional mobility, and geriatric rehabilitation preferred
