Full Job Description

Nexus at Belleville is seeking a Certified Occupational Therapy Assistant (COTA) to provide high-quality occupational therapy services in a skilled nursing setting. This role supports residents in improving independence, function, and quality of life through individualized treatment interventions under the supervision of a licensed Occupational Therapist (OT).

Responsibilities

  • Implement occupational therapy treatment plans developed by the supervising Occupational Therapist
  • Assist residents in improving activities of daily living (ADLs), including dressing, bathing, grooming, and mobility
  • Provide therapeutic interventions to address cognitive, physical, and functional limitations
  • Monitor patient progress and report responses to treatment to the supervising OT
  • Assist with restorative and maintenance therapy programs as needed
  • Document treatment sessions, progress notes, and resident responses accurately and timely
  • Collaborate with an interdisciplinary care team including nursing, rehabilitation staff, and physicians
  • Educate residents, families, and caregivers on adaptive techniques and safe functional practices
  • Support discharge planning and transition of care when appropriate

Qualifications

  • Associate’s degree from an accredited Occupational Therapy Assistant program
  • Active Illinois COTA license (required)
  • Current NBCOT certification (required)
  • Experience in skilled nursing or long-term care preferred
  • Strong understanding of ADL training, functional mobility, and geriatric rehabilitation preferred
  • New graduates encouraged to apply
