About the Opportunity

A well-established skilled nursing facility in Las Animas, Colorado is seeking a Certified Occupational Therapy Assistant (COTA) to join its rehabilitation team. This is an opportunity to make a direct impact on residents' independence and quality of life in a close-knit, community-focused facility in southeastern Colorado. You'll work alongside a collaborative interdisciplinary team in a setting where your contributions are visible and valued.

What You'll Do

  • Provide occupational therapy treatments under the supervision and plan of care established by a licensed Occupational Therapist (OTR/L)
  • Help residents improve or restore skills needed for daily living — ADLs, fine motor function, cognitive tasks, and safe mobility
  • Document treatment sessions, progress, and outcomes accurately and timely in the EMR (Point of Care / PCC or similar)
  • Communicate resident progress and concerns to the supervising OT and interdisciplinary care team
  • Educate residents, families, and caregivers on adaptive techniques and home exercise programs
  • Support discharge planning and contribute to care plan meetings as needed
  • Maintain compliance with state, federal, and facility regulations, including Medicare/Medicaid documentation standards

What You'll Bring

  • Graduate of an accredited Occupational Therapy Assistant program
  • Current NBCOT certification (COTA)
  • Active Colorado OTA license (or eligibility/willingness to obtain — license reciprocity candidates welcome)
  • CPR/BLS certification (or ability to obtain upon hire)
  • New grads welcome — SNF or long-term care experience a plus
  • Strong documentation skills and a team-first, resident-centered mindset
