Job Summary

As a Chaplain, your primary mission is to assess and address the spiritual needs of hospice patients and caregivers across all age groups. You will develop individualized plans of care, offer crisis intervention, and facilitate services of prayer or ritual that respect the diverse philosophies and beliefs of those we serve.

Key Responsibilities

  • Patient Support: Conduct spiritual assessments and develop a comprehensive plan of care (POC) tailored to the patient and family. 
  • Counseling & Rituals: Provide individual counseling, crisis intervention, and lead worship or rituals as appropriate. 
  • Collaboration: Work closely with the IDT and local clergy to resolve spiritual or ethical issues and update care plans. 
  • Bereavement Care: Make visits prior to death and remain available to support families during the bereavement period. 
  • Documentation: Complete and submit all clinical documentation in a timely manner to track patient responses and interventions.

Qualifications

  • Education: Bachelor’s degree required; Master of Divinity or Theology preferred 
  • Affiliation: Must be in good standing with your denomination or religious affiliation. 
  • Experience: At least one (1) year of experience as a Chaplain is required. 
  • Skills: Strong interpersonal skills and a working knowledge of community spiritual resources. 
  • Requirements
    • Valid Texas Driver’s License, reliable transportation, and auto liability insurance. 
    • Ability to pass a criminal background check. 
    • Proof of TB testing and Hepatitis acceptance/declination.

Working Conditions

This position involves traveling locally to visit patients in their homes. You should be comfortable working in various conditions, including occasional exposure to infectious diseases and unpleasant weather. The role requires prolonged standing, walking, and the ability to make quick decisions under stressful conditions to meet patient needs.
