Job Summary:

GENERAL DESCRIPTION
The Cherokee Speaker Home Health Provider collects information regarding the services and needs of first language speakers and ensures they receive available services. This position maintains a daily log of requests and makes home visits to provide basic medical assistance to citizens.

Job Duties:

ESSENTIAL DUTIES AND RESPONSIBILITIES
• Collects information from various sources regarding service needs of first language speakers residing within the Cherokee Nation reservation.
• Collaborates with team members and outside agencies to ensure the highest quality health care for citizens.
• Makes home visits to assess and evaluate needs and risks of the First Language Speaker population
• Assists first language speakers with personal care needs such as bathing, dressing, grooming and toileting.
• Ensures needs and preferences are communicated and heard by healthcare providers.
• Provides emotional support to patients and family members and encourages social engagement.
• Assists first language speakers in completing applications for services including Social Security Administration forms, Veterans affairs, etc.
• Maintains complete, accurate, and truthful records and reports on citizens, daily activities, program status, and other required information.
• Performs other duties, tasks, and services consistent with this position as assigned.


LEVEL OF RESPONSIBILITY
• This position does not directly supervise any individuals.
• Assignments received in broad terms with timelines and objectives set.

Qualifications:

QUALIFICATIONS
• High School Diploma or General Education Degree (GED).
• Six (6) months of experience in a related field.
• Advance Cherokee Speaker proficiency required.

PHYSICAL REQUIREMENTS
While performing the duties of this Job, the employee is regularly required to talk or hear. The employee is frequently required to stand; walk and sit. The employee is occasionally required to use hands to finger, handle, or feel; reach with hands and arms; climb or balance and stoop, kneel, crouch, or crawl. The employee must frequently lift and/or move up
to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.


CERTIFICATES, LICENSES, REGISTRATIONS
• Must possess a valid driver's license with a driving history verified through a motor vehicle report that meets requirements for Cherokee Nation underwriting rating.

• Possess or ability to obtain a certified nursing assistant (CNA) certificate within one (1) year of hire and maintain throughout employment.


OTHER REQUIREMENTS
The employee must not be and will not be under sanction by the United States Department of Health and Human Services Office of the Inspector General (OIG) or by the General Services Administration (GSA) or listed on the OIG's Cumulative Sanction Report, or the GSA's List of Excluded Providers, or listed on the OIG's List of Excluded Individuals/Entities (LEIE).
