The Opportunity

We are building a next-generation home care operating platform focused on creating a fundamentally better experience for patients, families, nurses, and caregivers.

Home care is a $130B+ industry, roughly half of which is private-duty care, yet the market remains highly fragmented and operationally inefficient. Families struggle to navigate care, caregivers often lack the training and support they need, and much of the industry continues to operate on legacy infrastructure and business models. We believe there is an emerging and rapidly growing demand for a higher quality level of at-home personal care services, and that now is the time to transform these services by empowering nurses to lead this care with a more clinical approach. Nurses have always understood best what a patient needs, and today's technology can enable them to steward caregivers to do better work than ever before.

Context

25m Health is a healthtech venture studio built in partnership with LifePoint Health, Scion Health, and Apollo Global Management. We build healthcare companies from the ground up and invest in early-stage startups. Our platform marries an expansive clinical footprint and expertise with builders and operators to partner with aspiring entrepreneurs and startups to accelerate their path to market. 25m Health's in-house platform team, based in Nashville and New York City, spans engineering (full-stack, security, data science), strategy (fundraising, product, research), design (UI/UX, branding, marketing), and back-office (hiring, legal, accounting, tax).

We are seeking a Chief Executive Officer (CEO) to lead the development and launch of this new incubation focused on building a new model for home care. As CEO, you will set the early vision, launch the first practices on the platform, build the operational model required to support them, and establish the foundation to scale Newco across markets.

You will partner closely with the 25m Health team, experienced operators and executives with a track record of company creation, to refine the model, build the technology and operational infrastructure, develop the go-to-market strategy, and launch Newco's first markets. 25m Health will also provide the foundational capital and continued support to help you execute on that vision as you transition into the CEO role in the new business.

What You'll Do

• Build from 0 to 1: Define Newco's vision and business model, translating insights from nurses, caregivers, patients, and families into an initial operating model, product roadmap, and growth strategy.
• Launch the business: Lead Newco's first market launches, developing the partnerships, operations, and go-to-market playbook required to build early traction and expand.
• Build the operating model: Design the infrastructure required to deliver exceptional care while creating a more efficient and scalable organization.
• Shape the care experience: Work closely with clinical leaders to build a differentiated model centered on quality, accountability, and an exceptional experience for patients and families.
• Technology enablement: Partner with 25m Health's engineering and product teams to determine how software, AI, and automation can transform the experience and economics of delivering care in the home.
• Financial leadership: Develop and manage Newco's financial model, including practice-level economics, pricing, and long-term company economics; use early operating data to refine the model and build a compelling path to scale.
• Company building & fundraising: Recruit and lead the founding team, establish the culture and operating cadence of the company, and work with 25m Health to raise external capital as the business reaches key milestones.

Who You Are

• 7+ years of experience in healthcare or healthtech, ideally with experience in home care, senior care, healthcare services, or another operationally complex healthcare business.
• Proven track record as a 0-to-1 builder, comfortable operating in ambiguity and creating structure from scratch.
• Strong operator who has built or scaled complex, people-intensive operations across multiple markets or locations.
• Deep understanding of the home care ecosystem, including patients and families, caregivers and nurses, referral channels, regulation, and the operational realities of delivering care in the home.
• Experience launching new markets, business lines, or healthcare services and developing repeatable operating playbooks that can scale.
• Entrepreneurial, analytical, and execution-oriented; equally comfortable recruiting the first nurse owner, talking to a patient referral partner, designing an operational workflow, reviewing unit economics, or helping shape the product roadmap.
• Strong company builder with the ability to recruit exceptional people, set a high bar, and create a culture of ownership and execution.
• Capable of developing a compelling company vision and translating early traction into a fundraising story for institutional investors.
• Passionate about creating a fundamentally better model for nurses, caregivers, patients, and families, with a bias toward action and continuous learning.
• Attention to detail and efficiency-focused. You love the challenge of turning complex, fragmented systems into simple, scalable operations.

Compensation

You will work with us on a full-time basis for a period of 3-4 months before stepping into a leadership role in Newco.

$150,000-$175,000 annual salary, commensurate with experience (may vary based on experience, location, and other factors). This is an independent contractor engagement paid on a pro-rated basis.

Founder-level equity stake in the business.
