Chief of Care Management

A healthcare organization is seeking an experienced Chief of Care Management to provide executive leadership and strategic direction for its care management workforce. This senior leadership role will focus on strengthening care management operations, developing teams, improving outcomes, and ensuring alignment between leadership and frontline operations.

The Chief of Care Management will provide high level oversight to the VPs of Care Management and the broader care management workforce. This individual will use data and analytics to evaluate performance, identify trends, improve processes, and establish measurable accountability across regions.

The ideal candidate is a strong and inspiring leader with extensive human services and IDD experience, a solid understanding of OPWDD systems, and the ability to drive initiatives from identification through implementation. This role requires someone who can maintain a strategic perspective while remaining closely connected to day to day care management operations.

Key Responsibilities:

• Provide executive leadership and strategic direction for the care management workforce

• Oversee and support VPs of Care Management and care management teams

• Develop and implement strategies to strengthen the workforce and promote person centered care

• Establish clear expectations, accountability, and performance standards across teams

• Use data and analytics to monitor outcomes, identify trends, and guide operational decisions

• Identify opportunities to improve and standardize care management processes

• Lead problem solving and improvement initiatives from identification through resolution

• Ensure alignment between care management teams and broader organizational objectives

• Coordinate with other departments to drive organization wide improvements

• Serve as a strategic liaison with OPWDD and other relevant agencies for escalated matters

• Participate in executive oversight, strategic initiative, and other high level meetings

• Ensure timely responses and resolution of inquiries and operational issues

• Lead special projects and provide operational support and coverage when needed

• Ensure compliance with OPWDD regulations and organizational policies

• Foster a positive, supportive, and accountable work environment that encourages professional growth

Qualifications:

• 10+ years of related human services experience required

• 5+ years of IDD experience required

• 3+ years of management experience required

• Strong understanding of OPWDD systems, compliance, quality, and outcome driven care

• Proven ability to lead and influence teams at multiple levels of an organization

• Strong analytical skills with experience using data to measure outcomes and improve performance

• Excellent communication and organizational skills

• Results driven with a strong focus on accountability and continuous improvement

• Strong technology skills and ability to effectively utilize relevant systems and tools

• Ability to manage sensitive and confidential information professionally

• Master's degree in Health Administration, Human Services, Social Work, Nursing, or a related field preferred

Salary: $150,000 to $200,000/year
