As a Children and Pregnant Women (CPW) Case Manager, you will play a vital role in coordinating care for pregnant women and children, ensuring they have access to the medical, social, and community resources needed to improve health outcomes.

What You’ll Do & Bring

As a CPW Case Manager, you will provide comprehensive case management services and coordinate care across clinical and community settings to support patients throughout their healthcare journey.

Key Responsibilities:

  • Manage care coordination for high-risk maternal-fetal medicine (MFM) patients through 12 months postpartum and pediatric patients with chronic conditions
  • Navigate referrals and initiate CPW services for both maternal and pediatric populations
  • Collaborate with providers and care teams to ensure patients follow individualized care plans
  • Conduct comprehensive case management from assessment through follow-up and program discharge
  • Screen for non-medical drivers of health and connect patients to appropriate community resources
  • Participate in clinical huddles and collaborate with operational and care teams
  • Conduct coordination visits to support patient adherence and improved health outcomes
  • Engage in community outreach and build relationships with partner organizations
  • Maintain knowledge of available healthcare and community resources to support patient needs
  • Perform additional duties as assigned within scope

What You Bring:

  • Licensed Registered Nurse (RN) or Licensed Social Worker (LSW) in the State of Texas
  • Active enrollment (or ability to enroll) as a Texas Medicaid provider
  • Completion of HHSC Case Management Training (or willingness to complete upon hire)
  • For RNs: Associate, bachelor’s, or advanced degree in nursing
  • For associate-degree RNs: Minimum two years of relevant experience or supervised practicum with children and/or pregnant women
  • For Social Workers: Appropriate licensure, including requirements for independent practice if applicable
  • Experience assessing psychosocial and health needs and coordinating community referrals
  • Strong communication, collaboration, and interpersonal skills
  • Ability to work with diverse populations and manage multiple priorities
  • Commitment to patient advocacy, care coordination, and quality improvement

Preferred:

  • Bilingual in English and Spanish
  • Prior experience in case management for maternal and pediatric populations
