Children's Long-Term Support Case Manager

Reporting to the respective program supervisor, the Case Manager - CLTS serves as a point of contact for participants, customers, and/or clients performing related duties toward the objectives and goals of the division.  

A successful candidate will be a team-oriented, organized, and compassionate professional who enjoys building relationships and having meaningful conversations with families about available services and supports. If you’re a strong communicator who can connect with families, understand their needs, and work collaboratively to help them find the right resources, we’d love to hear from you!

Hours:  Monday-Friday 8AM - 4:30PM with possibilities of comp time
Remote Work:  After 6 months of employment, you are eligible for one day per pay period to work from home.
