Reporting to the respective program supervisor, the Case Manager - CLTS serves as a point of contact for participants, customers, and/or clients performing related duties toward the objectives and goals of the division.
To be successful in this role during the first year, candidates should be comfortable working closely with children with disabilities and their families, building positive and supportive relationships. Success also requires being a collaborative team member who contributes to a larger multidisciplinary team. The ideal candidate is adaptable, enjoys a mix of office and community-based work, and is confident conducting home visits as part of providing services and support.
