Position Summary
Woodlands Medical Specialists is seeking a compassionate, detail-oriented, and highly skilled Registered Nurse to join our growing outpatient clinical team in the Chronic Care Management Department. The ideal candidate will have at least 1-3 years of nursing experience and demonstrate strong clinical judgment, excellent patient communication skills, and the ability to effectively collaborate with multiple providers in a fast-paced clinic environment.
This role combines direct patient care, chronic disease management, clinical research support, patient education, triage services, and administrative responsibilities to ensure high-quality patient outcomes and exceptional patient experience
Qualifications
- Current Registered Nurse (RN) license in good standing
- Minimum of 1-3 years of nursing experience in a clinical, ambulatory, or healthcare setting
- Strong clinical assessment and critical-thinking skills
- Excellent interpersonal and communication abilities
- Ability to prioritize tasks and manage multiple responsibilities efficiently
- Experience with electronic medical records (EMR) systems preferred
Essential Clinical Responsibilities
- Conduct telephone and in-person nursing triage to assess patient needs and coordinate appropriate care
- Perform comprehensive patient assessments, including obtaining vital signs and health histories
- Provide patient education focused on chronic disease management, health promotion, and disease prevention
- Administer treatments and medications in accordance with provider orders and established protocols
- Perform venipuncture and laboratory specimen collection
- Initiate and maintain peripheral intravenous (IV) access
- Assist providers with examinations, procedures, and treatments
- Support clinical research activities, including patient screening, documentation, data collection, and protocol compliance
- Monitor patient status and communicate significant findings to providers
- Assist patients with prescription refill requests and medication-related questions
- Ensure compliance with infection prevention and patient safety standards
Administrative Responsibilities
- Accurately document patient encounters and nursing interventions in the EMR
- Schedule appointments, coordinate referrals, and facilitate continuity of care
- Process medication refill requests and prior authorizations
- Manage patient communications through phone calls, electronic messaging, and follow-up outreach
- Collaborate with providers and staff to improve workflows and operational efficiency
