    Description

    POSITION SUMMARY: Perform the duties required to coordinate and monitor nursing services for the Chronic Disease Department. Provide direct care with patients, work with Health Coaches and BCCCP Follow-Up Assistant.

    POSITION QUALIFICATION REQUIREMENTS

    Education: Bachelor of Science Degree in Nursing.

    Experience:

    • Five (5) years'' experience in ambulatory care, hospital and /or community health nursing.
    • Experience in ambulatory care, hospital or community health nursing.

    • Good organizational, analytical, and problem-solving skills.
    • Excellent verbal and written communication skills.
    • Able to work with people with diverse ethnic backgrounds.
    • Strong computer skills including the use of word process, spreadsheet and e-mail software.
    • Able to work on multiple tasks simultaneously.

    Residency and Transportation Requirement: Must possess a valid driver''s license and automobile with insurance coverage as required by the State of Missouri.

    License(s)/Certification(s) Required: Licensed RN in Missouri and CPR certified.

    FUNCTIONS OF THE JOB

    Essential Functions

    • Work with health coaches'' compliance to established policy and procedures with daily work schedules.
    • Recruit, patients with Chronic Disease conditions and work with qualified health coaches and BCCCP Follow-up Assistant.
    • Coordinate and implement training, educational classes and in-services for staff in chronic disease management, patients and JACHO standards.
    • Assist in the development and implementation of policies and procedures for Chronic Disease department and other areas as required.
    • Coordinate and oversee special programs as assigned.
    • Prepare and maintain needed records and reports.
    • Prepare and revise educational materials for clients with chronic diseases.
    • Complete outreach/in reach and follow up activities provided through BCCCP.
    • Participate as a member of the Health Disparities Collaborative teams dealing with chronic diseases.
    • Work in coordination with health center nurse coordinators and supervisors to monitor and maintain adherence to JCAHO and other accrediting body standards.
    • Represent Affinia Healthcare in the community and make presentations as requested.
    • Monitor the goal achievement of Healthy Heart Program and other grants and contracts involving coaching of clients with chronic diseases.
    • Participate in the planning and evaluation of new and existing programs.
    • Perform other duties as assigned.

    Marginal Functions: Orientates volunteers, physicians, nurse instructors and students.
