Job Title: Community Health Worker (CHW)

Compensation: $26.37 per completed visit Weekly Pay + Travel Reimbursement
About the Role:
Gotham Companies is seeking dedicated Community Health Workers (CHWs) to deliver high-touch, field-based care management across NYC and Westchester. In this role, you will conduct in-person home visits, connect members to vital community resources, and partner closely with clinical teams to improve overall health outcomes.
Key Responsibilities:
Complete in-person member visits and assessments (approx. 30 minutes per visit).
Maintain the required target of completed visits per week (with potential for 2535+ visits).
Refer members to local community programs, social services, and housing/food assistance.
Educate members on chronic condition management, wellness, and self-care.
Accurately track and log daily visit activity using Excel spreadsheets and care management systems.
Assist members with scheduling transportation via MAS during in-person visits as needed.
Qualifications & Requirements:
Education: High School Diploma or GED required (Associates or Bachelors degree preferred).
Experience: 3+ years of casework, community outreach, healthcare related and/or social work experience required.
Technical Skills: Proficient in MS Office (especially Microsoft Excel) and comfortable navigating electronic care management systems.
Travel: Willingness to travel to member homes within your designated coverage area.
Compensation & Benefits:
Pay Rate: $26.37 per completed visit (Weekly Pay).
Earning Potential: $900+ weekly based on visit volume.
