Position Overview
The Special Education Transition Teacher provides individualized instruction and community-based programming for young adults preparing for life after high school. This position focuses on developing independent living, vocational, employment, and self-determination skills that prepare students for successful adult lives. Teachers create meaningful, real-world learning experiences that promote independence, confidence, and self-advocacy
Responsibilities
Plan and deliver individualized instruction aligned with students IEPs and transition goals
Design and lead Community-Based Instruction (CBI) in settings such as job sites, grocery stores, banks, and public transportation
Teach independent living skills including money management, cooking, hygiene, home management, and community navigation
Create a respectful, age-appropriate learning environment
Support social-emotional development using positive behavior supports and crisis intervention when needed
Coordinate job coaching, work-based learning, and internship experiences
Teach employment skills including resume writing, interviewing, workplace communication, and professional expectations
Collaborate with local businesses, community organizations, the Bureau of Rehabilitation Services (BRS), Department of Developmental Services (DDS), and other agencies to support transition planning
Collect and analyze student data to guide instruction
Maintain gradebooks, student records, and quarterly progress reports
Serve as an IEP Case Manager
Develop and monitor transition-focused IEP goals
Complete transition assessments, person-centered planning, Summary of Performance (SOP) documentation, Functional Behavioral Assessments (FBAs), and Behavior Intervention Plans (BIPs) as needed
Participate in PPT meetings, annual reviews, parent conferences, and transition planning meetings
Collaborate with related service providers, job coaches, administrators, and community partners
Participate in school and community events
Qualifications
Required
Connecticut State Special Education Certification (065, 165, or 265)
Strong classroom, community-based management, and positive behavior support skills
Experience working with young adults with diverse learning, behavioral, social, and emotional needs
Valid drivers license and ability to accompany students during community-based instruction (or ability to obtain)
Physical Management Training (PMT) certification within 60 days of employment
Preferred
Bachelors or Masters Degree in Special Education
Experience with transition planning, vocational training, or job coaching
Knowledge of BRS, DDS, and post-secondary transition services
Experience with individualized instruction and data-driven decision making
