We're looking for a compassionate, organized, and proactive Client Success Manager to become the trusted point of contact for our clients and their families. In this role, you'll ensure every client receives outstanding service—from the very first phone call through every stage of their care journey.

You'll work closely with families, caregivers, nurses, schedulers, and our leadership team to deliver a seamless experience while helping seniors remain safe, independent, and comfortable in their own homes.

What You'll Do

As our Client Success Manager, you will:

  •  Build lasting relationships with clients and their families through proactive communication and exceptional service. 
  •  Guide new clients through the onboarding process, ensuring a smooth and welcoming start to care. 
  •  Conduct in-home and virtual client assessments and develop personalized care plans. 
  •  Ensure every service plan is complete, compliant, and in place before care begins. 
  •  Perform regular client wellness check-ins and quality assurance visits to ensure satisfaction and address concerns before they become problems. 
  •  Respond quickly and professionally to client questions, service requests, and concerns. 
  •  Coordinate changes in care needs and communicate updates to the appropriate team members. 
  •  Review and ensure client records remain accurate, complete, and compliant. 
  •  Manage incoming referrals and prospective clients from hospitals, physicians, senior communities, and marketing partners. 
  •  Maintain HIPAA compliance and agency documentation standards. 
  •  Participate in the agency's rotating on-call schedule to support clients and caregivers after business hours. 
What We're Looking For

Required Qualifications

  • 1–3 years of experience in home care, healthcare, senior living, case management, client services, healthcare coordination, or customer success is required.
  •  Outstanding communication and relationship-building skills. 
  •  Strong organizational skills with exceptional attention to detail. 
  •  Ability to prioritize multiple tasks in a fast-paced environment. 
  •  Professional, compassionate, and solutions-oriented mindset. 
  •  Comfortable using Microsoft Office and electronic documentation systems. 
  •  Valid driver's license and reliable transportation for client visits. 
  •  Ability to pass a criminal background check. 
Preferred Qualifications
  •  Experience in non-medical home care. 
  •  Experience with WellSky or similar home care software. 
  •  Knowledge of HIPAA and Illinois home care regulations. 
  •  Bilingual candidates are encouraged to apply.
