Client Care Coordinator

Position Summary

The Client Care Coordinator plays a vital role in guiding clients and their families through the home care process while ensuring Safe Harbor Home Care caregivers are trained, supported, and consistently delivering exceptional service. This position serves as the bridge between clients, families, caregivers, healthcare partners, and internal teams to promote continuity of care, client satisfaction, caregiver success, and operational excellence.

The Client Care Coordinator demonstrates ownership and accountability in five core areas: Assessments and Client Support; Hospital/SNF Follow-Up and Care Coordination; Quality Assurance and Reassessments; Client and Family Communication; and Caregiver Follow-Up and Regional Support.

1. Assessments and Client Support

· Conduct consultations with prospective clients and families to assess care needs and explain available home care services.

· Develop individualized, high-quality care plans tailored to each client's physical, emotional, and personal care needs.

· Present care plans to clients and family members and address questions or concerns regarding services.

· Facilitate completion and signing of service agreements and required admission documentation.

· Collaborate with the Scheduling Team to ensure timely staffing and seamless initiation of care services.

· Serve as a resource during onboarding of new clients.

· Provide field support as needed to ensure continuity of care and client satisfaction.

· Assist with staffing coverage by filling gaps in care when necessary.

· Deliver direct client care when operational needs require.

2. Hospital / Skilled Nursing Facility Follow-Up and Care Coordination

· Maintain communication with hospitalized clients, family members, hospital staff, SNFs, social workers, and case managers.

· Coordinate home care services and care transitions for clients discharging from hospitals, rehabilitation centers, and SNFs.

· Collaborate with physicians, discharge planners, and healthcare providers.

· Support efforts to reduce avoidable hospital readmissions.

· Monitor client status during care transitions and communicate updates.

3. Quality Assurance, Reassessments, and Assignment of Benefits

· Conduct routine quality assurance calls and in-person visits.

· Perform scheduled care plan reassessments and update documentation.

· Ensure caregiver services align with established care plans, company standards, and regulatory requirements.

· Identify opportunities for service improvement.

· Obtain and maintain required Assignment of Benefits documentation.

· Ensure annual renewal and completion of AOBs for VA clients.

4. Client and Family Communication

· Maintain regular communication with clients and family members.

· Address client and family concerns promptly, professionally, and compassionately.

· Serve as the primary liaison between clients, families, caregivers, and scheduling staff.

· Provide updates regarding staffing, care plan adjustments, and service changes.

· Encourage satisfied clients and families to provide reviews and testimonials.

5. Caregiver Follow-Up and Regional Support

· Conduct caregiver training, coaching, and field observations.

· Monitor caregiver performance and provide feedback.

· Support newly hired caregivers through regular follow-up and mentoring.

· Ensure caregivers understand client-specific care plans and expectations before assignments.

· Identify educational opportunities and provide additional training.

· Work closely with caregivers who have limited schedules or reduced hours.

· Assist with caregiver-client matching and improve staffing consistency.

Qualifications

Education and Credentials

· High School Diploma or GED.

· Active Certified Nursing Assistant (CNA), Certified Home Health Aide (CHHA), or Home Care Aide (HCA) registration/certification.

· Valid driver's license, reliable transportation, and proof of automobile insurance.

Experience

· Minimum one year of caregiving experience in home care, home health, assisted living, memory care, skilled nursing, or related healthcare setting.

· Experience conducting client assessments and developing non-medical care plans preferred.

· Experience working with hospitals, skilled nursing facilities, discharge planners, social workers, or case managers preferred.

· Experience training, mentoring, or supporting caregivers preferred.

· Strong interpersonal and relationship-building skills.

· Excellent verbal and written communication abilities.

· Exceptional customer service and conflict-resolution skills.

· Working knowledge of ADLs, dementia care, companionship services, and home safety practices.

· Ability to train, mentor, and coach caregivers.

· Proficiency with electronic documentation systems and Microsoft Office applications.

· Competency in using Hoyer Lift and Sit-to-stand devices.

Physical Requirements

Ability to sit, stand, walk, bend, stoop, kneel, and reach throughout the workday. Ability to lift up to 25 pounds and travel throughout assigned service areas.

Working Conditions

Combination of office, client home, hospital, skilled nursing facility, and community-based settings. Frequent local travel required.
