    Client Growth & Care Partnership Manager

    Hybrid Business Development & External Care Coordination

    Location: Polk County, Florida

    Schedule: Full-Time

    Work Arrangement: Hybrid Field & Office

    Compensation:$65,000–$70,000 Base Salary + Quarterly Performance Bonus + Mileage Reimbursement + Benefits


    Our client, a well-established home care organization serving Polk County, Florida, is seeking a relationship-driven Client Growth & Care Partnership Manager to help grow the business while ensuring every client and family receives an exceptional experience from the first conversation through ongoing care.

    What Makes This Role Unique

    Own the Entire Client Journey

    This role combines business development, relationship management, and external care coordination.

    You’ll help generate new client opportunities, build trusted community partnerships, complete in-home consultations, and remain actively involved after services begin to ensure clients and families feel supported.

    Become the Face of the Organization in Polk County

    You’ll spend time in the community developing relationships with referral partners while also working directly with clients and families in their homes.

    You’ll have the opportunity to become a trusted resource for both professional referral sources and families navigating home care decisions.

    Key Responsibilities

    Business Development & Community Outreach

    • Develop strong relationships with physicians, hospitals, rehabilitation centers, senior living communities, and other referral partners
    • Conduct community outreach throughout Polk County
    • Generate new client opportunities
    • Maintain and grow an active referral pipeline
    • Complete in-home consultations with prospective clients and families
    • Convert qualified referrals into new clients
    • Educate referral partners and families about available home care services
    • Maintain consistent follow-up with referral sources and prospective clients
    • Identify new opportunities to increase census and regional revenue


    Client Experience & Care Coordination

    Once clients begin services, you’ll continue owning the relationship.

    Responsibilities include:

    • Conduct supervisory visits
    • Complete care assessments
    • Support clients and families throughout the care experience
    • Monitor caregiver quality and service delivery
    • Identify and resolve concerns before they become larger problems
    • Help ensure continuity and quality of care
    • Increase client satisfaction and retention
    • Communicate proactively with families regarding changes, concerns, or service needs
    • Partner with internal team members to ensure an exceptional client experience

    Relationship Management

    • Become a trusted advisor to both families and referral partners
    • Build long-term relationships based on trust, responsiveness, and follow-through
    • Communicate proactively and professionally
    • Educate families and referral partners
    • Create confidence throughout the client journey
    • Strengthen referral relationships that lead to ongoing business growth
    • Represent the organization professionally throughout the Polk County community

    Qualifications

    Required

    • Strong relationship-building and communication skills

    • Ability to build trust quickly with clients, families, and referral partners
    • Strong organizational and follow-up skills
    • Ability to manage multiple priorities independently
    • Comfortable working in both field and office environments
    • Professional, polished, and dependable
    • Strong problem-solving abilities
    • Ability to work collaboratively with internal operational and care teams
    • Valid driver’s license and reliable transportation
    • Ability to travel throughout Polk County as needed

    Preferred

    • Experience in home care, home health, hospice, senior living, or another healthcare environment
    • Business development, sales, or community outreach experience
    • Care coordination, case management, nursing, social work, or therapy experience
    • Experience completing in-home consultations or assessments
    • Experience developing referral partnerships
    • Familiarity with senior care services and community resources
