Position Summary:
The Clinical Pharmacist provides clinical pharmacy support to a well-defined patient population through the utilization of advanced skills in a focused area of pathophysiology/pharmacology.
Responsibilities:
- Knowledge of all pharmacist activities and performs such duties as requested.
- Provides support/consultative services to physicians, nurses and other healthcare providers regarding drug therapy decisions. Provide comprehensive inpatient/outpatient drug therapy management through physician consult.
- Responsible for quality assessment, quality improvement, and written policies and procedures within specialty area.
- Responsible for overall reporting/documentation of suspected adverse drug reactions and medication occurrence.
- Responsible for ensuring the optimal provision of drug therapy for the purpose of achieving outcomes which improve a patient’s quality of life.
- Manages any unit specific patient care problems relative to pharmacy.
- Responsible for patient education as applicable within the specialty area.
- Responsible for compliance with all state and federal pharmacy regulations and requirements.
- Demonstrates knowledge and competency in providing for age-specific needs of the population served.
- Participates in community education programs to provide drug information. Participate in the health system employee education programs to provide drug information.
- Development of clinical pathways via multi-disciplinary approach.
- Precepts pharmacy students in specialty area.
- Serves as a member of Covenant Health Systems Formulary Committee and FSRMC Pharmacy and Therapeutics Committee. Present drug monographs or drug information as requested by the committee. Monitor non-formulary medication usage and provide recommendations for alternative therapy when appropriate.
- Responsible for rotation of 24 hour call for clinical services.
- Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
- Performs other duties as assigned.
Qualifications:
Minimum Education:
None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.
Minimum Experience:
Demonstrated written and/or verbal communication skills necessary. Ability to perform drug usage evaluations relative to their area of expertise and ability to research and disseminate drug therapy information to health care personnel.
Licensure Requirement:
Current TN pharmacist license.
