Description:
The Clinic Care Navigator supports Medical Management by assisting with care coordination, patient outreach, and administrative functions that do not require RN-level intervention. This role focuses on identifying and guiding patients, particularly those at higher risk, to improve health outcomes, enhance patient and provider experience, and reduce overall healthcare costs. This is a fully on-site position based in Las Vegas, NV.

What You Will Do:

  • Coordinate patient care activities between Utilization Management (UM) and Case Management (CM) teams
  • Conduct outreach to patients, including post-discharge follow-up calls (acute and post-acute settings)
  • Identify and support high-risk and non-high-risk patients through data and case finding tools
  • Manage ER utilization reports and implement interventions to reduce inappropriate ER visits
  • Assist with gathering clinical information, including referrals, claims history, and documentation
  • Provide administrative and operational support to the Medical Management team
  • Maintain accurate and timely documentation within EMR systems
  • Develop spreadsheets and tracking tools to support care management workflows
  • Assist patients with accessing community resources and closing gaps in care
  • Ensure compliance with patient confidentiality standards

What You Will Bring:

  • High School Diploma required
  • Background as a Medical Assistant or current nursing student preferred
  • Experience in a managed care setting preferred
  • Knowledge of medical terminology required
  • Strong computer skills including Microsoft Office (Word, Excel) and EMR systems
  • Excellent communication, organizational, and interpersonal skills
  • Ability to handle sensitive patient information with confidentiality
  • Familiarity with clinical workflows, vital signs, and basic patient care concepts
