    Job Description

    JOB SUMMARY:
     
    The Medical Assistant Care Coordinator is responsible for patient care within the setting of the clinic environment.  The Care Coordinator position also assist patient with managing their health care needs remotely (via telephone, or video calls).  They are trained and capable of performing patient care for the age groups newborn to 105 years old.
     
    The Medical Assistant Care Coordinator reports directly to the Director of Population Health.
     
     
    QUALIFICATIONS:
     
    1. Education:
      1. High School graduate.
      2. Good customer service skills.
      3. Prefer graduate of a Medical Assistant program, but not required. 
     
    1. Licensure/Certification:
      1. Certification as a medical assistant preferred.
      2. Basic Life Support (BLS) required.
    2. Work Experience:
      1. Minimum 1 year working as a medical assistant in a practice, preferred.  Graduation from a medical assistant program may be substituted in lieu of the 1-year work experience.
     
     
    RESPONSIBILITIES/JOB DUTIES:
     
    1. Take vital signs; obtain medical history, gives injections and immunizations as ordered by the clinic providers.
     
    1. Maintain required logs and information needed for quality control.  Performs Medicare Annual Wellness (AWV).
     
     
     
    1. Work quality measures for improvement of patient care measures for MIPS/MACRA, HEDIS, individual payer source reports, registries in Meditech, and other resources to improve quality measures for the Medical Group, provider and patients.
     
    4.  Collaborates with clinic providers and staff to strengthen data quality, collection, analysis, and reporting of health information.

    1. Possesses and displays a strong knowledge and understanding of the standards of
    practice for their position including, if and when applicable, commensurate computer skills; population specific and general competencies; organizational skills, time and project management.
     
    1. Possesses a vigorous and enthusiastic desire to learn and to grow both personally
    and professionally, and to improve one's skills in order to deliver the best possible care for our customers.
     
    12. Perform other duties as assigned.

    1. Exposed to all patient care elements.
     
    1. Must assist with lifting and moving of patients as needed.
