Job Summary
Under the direction of the Primary Care Provider (PCP) and the supervision of the Clinic Supervisor, the role is responsible for providing nursing care to participants in the clinic. This includes support with clinic overflow, checking vital signs, performing POC testing, managing and administering medications, and providing wound care and any other clinical needs in accordance with the scope of practice. The position supports clinic staff by managing ongoing care, complex clinical situations, assisting with procedures as needed, and ensuring efficient operations of the clinic. Collaboration with colleagues and stakeholders is essential to promote values, team culture, and mission.
Job Responsibilities
- Perform and document ordered procedures and care in the clinic, including medication management and administration as directed by PCP.
- Remain alert to input from team members, participants, and caregivers, and update the RN Case Manager and/or other clinic staff promptly of any changes in participants' condition or medical status.
- Support clinic staff to ensure smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.).
- Dispense ordered medications to participants and provide education regarding medications.
- Instruct and/or provide as-needed teaching to participants, family, and caregivers regarding disease processes, self-care techniques, and prevention strategies.
- Provide backup coverage to other nursing functions as assigned.
- Perform PGX testing and provide clinical support to Medical Assistants performing procedures in the clinic.
- Process Specialty Pharmacy medication orders and escalate to MD as needed.
- Address participant medication issues and questions while in the clinic.
- Communicate medication incidents or errors at Morning Briefing meetings and ensure follow-up.
- Coordinate with providers on any medication order changes from same-day procedures and manage medication changes as directed by external providers.
- Perform pre-enrollment medication evaluation.
- Provide coverage as needed in various roles to support the care team in delivering and coordinating patient care.
Administrative Duties
- Maintain clinical trackers such as TB Screening, Flu, Pneumovax, and Shingles.
- Monitor and maintain Emergency Cart monthly log and restock as needed.
- Stock the Meds ATM and destroy expired medications in accordance with policy and update Medication Destruction log.
Required Skills & Experience
- Minimum of one year of nursing experience in a clinical setting with a frail or elderly population.
- Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations.
- Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
- Excellent clinical, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members.
- Highly organized to accurately perform numerous tasks for multiple patients.
- Ability to work independently with minimal supervision.
- Demonstrated ability to prioritize in a fast-paced environment.
- Experience and competency working with people from diverse backgrounds and cultures.
- Commitment to unlocking the full potential of vulnerable seniors.
Preferred Skills & Experience
- Bilingual English/Spanish preferred.
Schedule Notes
Monday to Friday, 8:00 AM to 4:30 PM. From time to time, it may require overtime. Candidates must have reliable transportation and be comfortable working in an outpatient clinic setting. The role involves effectively managing triage, urgent clinical concerns, medication administration, wound care, and participant education. Strong critical thinking and excellent communication skills are essential, as this role serves as a key clinical resource for both participants and the interdisciplinary care team. Experience in acute care is highly valued; however, experience in primary care, ambulatory care, geriatrics, community health, or care coordination is also a good fit. Candidates with an acute care background such as ICU, telemetry, or medical-surgical nursing are highly desired. Experience assessing changes in condition, prioritizing care, and responding to urgent situations is particularly important.
