Job Description
Responsible for assuring the safety, health, and well-being of residents in their apartments and cottages. Addresses any concerns or needs indicated for the proper care of residents, assuring the philosophy and goals of the community are being met by assisting residents to maintain their highest level of independence. Supervise the nursing care given to residents requiring assistance with activities of daily living. Coordinates discharge planning of residents between the various levels of care. Maintains close working relationship and open lines of communication with all departments.
EDUCATION, EXPERIENCE, CERTIFICATION AND LICENSURE REQUIREMENTS
- Graduate of an accredited school of nursing with a diploma or degree.
- Licensed as a RN in Virginia or a compact multistage License required.
- CPR/AED certification required.
- 0-2 years of practical nursing experience.
Job Duties (Essential)
- Manages all day-to-day operations within the community. Initially sets up resident medical records and continually updates information.
- Coordinates medical services in the community. Conducts assessments of residents that need special care or assistance with activities of daily living as the need arises.
- Plans, supervises, and implements daily resident care and determines care responsibilities. Assists Director of Nursing with establishing and implementing clinic policy and procedures.
- As a member of Resident Care Committee, reports any special concerns or needs of the residents; conducts yearly reviews to be discussed at Resident Care Committee meeting.
- Follows up on hospital admissions from apartments and reports to Resident Care Committee.
- Performs individual resident care as required by a professional nurse (such as medications, dressings, and treatments).
- Directs and supervises resident care as performed by certified nursing assistants and nurses.
- Member of Safety Committee reporting any safety hazards as it applies to apartment and cottage residents.
- Assists the Nursing Supervisor in Healthcare Services, when necessary, in overseeing functions of the community.
- Assists physician in Health Clinic with residents and management of employee health services (new hire processing/ health screening).
- Responds to emergencies in apartments. Complete incident reports to document accidents, fire alarms, etc., and forwards to Chairman of Safety and Security Committee.
- Assesses employee and resident injuries and renders first aid or makes decisions for appropriate referral to doctor or ER.
- Assists with resident admissions to Assisted Living and Memory Support.
- Coordinates documented discharge plan with Social Services Director and Nursing Supervisor.
- Maintains resident statistics and resident care records.
- Maintains Resident Care Log.
- All other duties as assigned.
Qualifications
- Basic conceptual knowledge of theories, practices, and procedures within nursing, typically acquired through a nursing degree.
- Basic verbal and written communication skills required.
- Demonstrates working knowledge of common disease processes and treatments.
