Care Coordinator/Patient Navigator to work at a Facility in
Hampton Bays, NY. These are

Pay Rate: $26.59 / hour
Shift:Job Descriptions:- Coordinate patient care and appointments
- Help patients navigate healthcare services and resources
- Communicate with patients, families, and healthcare providers
- Schedule referrals, tests, and follow-up visits
- Educate patients about treatment plans and available services
- Monitor patient progress and address care needs
- Maintain accurate patient records and documentation
- Assist with insurance, referrals, and care coordination
- Identify barriers to care and help resolve them
- Collaborate with healthcare teams to ensure quality patient care
