Care Coordinator/Patient Navigator Duties & Responsibilities:

• 
  Coordinate patient care and appointments
  
• 
  Help patients navigate healthcare services and resources
  
• 
  Communicate with patients, families, and healthcare providers
  
• 
  Schedule referrals, tests, and follow-up visits
  
• 
  Educate patients about treatment plans and available services
  
• 
  Monitor patient progress and address care needs
  
• 
  Maintain accurate patient records and documentation
  
• 
  Assist with insurance, referrals, and care coordination
  
• 
  Identify barriers to care and help resolve them
  
• 
  Collaborate with healthcare teams to ensure quality patient care
  

Minimum Qualifications:

• 
  Ability to build rapport with patients
  
• 
  Compassionate and caring demeanor
  
• 
  Experienced as Care Coordinator/Patient Navigator
