The Stroke Program Clinical Data Coordinator is responsible for the collection, abstraction, analysis, and reporting of stroke-related clinical data to support quality improvement initiatives, regulatory compliance, accreditation requirements, and program development. The coordinator works collaboratively with multidisciplinary teams to monitor performance metrics, identify trends, maintain data integrity, and support ongoing stroke center certification and regulatory reviews.
This position plays a key role in advancing evidence-based stroke care through comprehensive data management, outcome monitoring, and quality reporting.
Principal Accountabilities:
Collect, abstract, and manage stroke program data from electronic health records, clinical registries, and internal databases.
Utilize various databases and reporting tools to generate queries, reports, dashboards, and data summaries.
Monitor and analyze quality indicators related to stroke care, including mortality, complications, readmissions, carotid interventions, EVD infections, and 30-day stroke outcomes.
Prepare monthly, quarterly, and ad hoc reports for stroke leadership, quality improvement committees, and regulatory agencies.
Create presentations and supporting documentation for Joint Commission reviews, accreditation surveys, and regulatory audits.
Conduct post-discharge follow-up calls with stroke patients discharged home and document outcomes.
Ensure accuracy, completeness, timeliness, and inter-rater reliability of clinical data submissions while resolving data discrepancies.
Track performance trends and provide actionable insights to support quality improvement initiatives.
Collaborate with clinical teams to maintain compliance with current stroke guidelines, quality measures, and certification standards.
Support educational initiatives and data management activities required for Comprehensive Stroke Center and Primary Stroke Center programs.
Assist with preparation for regulatory reviews, accreditation surveys, and program evaluations.
