Overview
Conducts and reviews UAS-NY comprehensive assessments of potential and existing members. Establishes an individual Plan Of Care based on members clinical, environmental, and social needs to support member safety in the community. Identifies solutions that promote high quality and cost-effective health care services in accordance with VNS Health Plans policies, state and federal regulations.. Manages requests for services from providers, members, and care management team. Works under general supervision.
Qualifications
Licenses and Certifications:
Current license to practice as a Registered Professional Nurse in New York State required Certified Case Manager preferred Valid driver's license or NYS Non-Driver photo ID card required as determined by operational/regional needs required
Education:
Bachelor's Degree in nursing or similar field required
Work Experience:
Minimum two years of clinical assessment, homecare or hospital experience preferred Excellent organizational and time management skills, interpersonal skills, verbal and written communication skills required Demonstrates strong relationship management skills, including a high degree of interpersonal communication l and non-aggressive assertiveness required Demonstrates successful conflict management skills and achieving "win-win" solutions required Working knowledge of Microsoft Suite Products e.g. Excel, Power-Point, Word required Knowledge of Medicaid and Medicare regulations required Working Knowledge of UAS-NY preferred
Pay Range per Visit:
If you are applying to the per diem per visit version of this job, the hiring range is as follows: $80 per visit.
Pay Range
USD $85,000.00 - USD $106,300.00 /Yr.
