Reports To: Resident Services Director
Summary: The Clinical Care Coordinator assists the Resident Services Director with all administrative tasks for the Resident Service Department.
Essential Functions:
- Develops RSA staffing schedule in conjunction with the Resident Services Director and inputs into Ulti
- Coordinates entry of nurses schedule into Ulti with Resident Services Designee
- Assembles resident records for new move ins
- Ensures that RSAs are completing 120-day resident assessments related to RSA tasks - prepare packets
- Enter resident service plans (monthly task log) into the Yardi and prompts Resident Services to activate incomplete task log
- Prints RSA assignment sheets from Yardi and provides to staff
- Coordinates family meetings with Resident Services tea using service plan
- Manages supplies including ordering for residents and Resident Services department
- Assists in interview and hiring process in conjunction with RSD- review applications, phone screen, coordinate interview, set up shadowing, coordinates references, assembles paperwork and completes Employee Status Change Form, etc.
- Update communication log, Voice friend, RSA Assignment Sheets and Wellness Number list with new employees
- Conduct pendant and wall alarm checks monthly
- Supports payroll with the daily reviewing of timesheets and entering missed punch forms/vacation requests into Ulti
- Complete payroll for Resident Services team bi-weekly
- Supports RSA performance management by consolidating time and attendance data for RSD
- Other duties as assigned and/or needed including Orientation support, training new Clinical Care Coordinators, etc.
Education/Experience/Licensure- Certification:
- High School education or equivalent required
- Medical Records or Medical Assistant certification preferred
Skills and Abilities:
- Computer skills required- solid knowledge of MS Office
- Organized
- Has sound working knowledge of policies in the Resident Handbook
- Possess excellent communication skills
- Possesses knowledge about the assisted living philosophy
- Demonstrates knowledge of the following Maplewood Senior Living Standards and Practices:
- OSHA guidelines, e.g., chemicals, personal care products, and items locked
- Use of Radios and keys
- Nametag and uniform
- Proper tracking of punching in and out for Resident Services Staff through Ultipro
- Alarm system
- Fire safety and elopement
- Has the ability to expertly multi-task
Compliance & Safety:
- Follows emergency procedures
- Understands the safety practices and procedures
Communication:
- Communicates effectively with supervisor and other staff
- Advise supervisor and appropriate manager in any changes in physical and mental health of resident immediately
Physical Requirements:
- Physically able to move at least 50 lbs. without assistance.
- Physical stamina and ability to assist residents or lift residents where needed and in emergencies
- Physically able to bend, reach and work in small areas
- Physically able to push and pull, equipment and furnishings
- Physically able to stand and/or sit for extended periods of time
Miscellaneous:
- Represents the community with a positive attitude and pride when interacting with potential residents and families
- Required to work weekends and holidays as assigned
- May be required to work on shifts other than the one for which hired
- May be required to work extended hours up to 16 hours per day
- Subject to exposure to infectious waste, diseases, conditions, etc. including HIV, AIDS and Hepatitis B & C viruses
- Could be subject to hostile and emotionally upset residents due to mental status
