Clinical Care Guide (RN)

We are a national staffing firm and are currently seeking a Registered Nursefor a prominent client of ours. This position is located in Tucson, AZ, 85718.Details of the position are as follows: 
 
Job Description: 

• Pay: $87,000-$90,000 annually to start!
• Direct hire position.
• Onsite in Tucson, AZ!
• Position will mainly be responsible for: Helping patients understand referrals, coordinating specialist appointments and diagnostic services, scheduling visits, and supporting patients throughout the referral process to improve their care.
• Responsibilities also include: Managing patient outreach programs, helping close gaps in care, educating patients with chronic conditions, handling office processes, coordinating service requests, and sharing referral information with the care team.
• Position will also be responsible for: Building relationships with healthcare providers and community organizations, supporting a positive patient experience, and performing other assigned duties.

• RN certification in Arizona is required!
• Bachelor’s degree in nursing is preferred.
• CPR Certification is required!
• Experience with electronic medical records is preferred.
• Experience with case management, or a Case Management Certification (CCM) is preferred.
