Job Summary
We are seeking a Clinical Coordinator to oversee scheduling, care coordination, and clinical operations for a home health agency serving patients across the Texas market. The coordinator will serve as the primary liaison between field clinicians, patients, physicians, and the administrative team.
Key Responsibilities
- Coordinate scheduling of home health visits for nursing, therapy, and aide staff
- Serve as point of contact for patients, families, and referring physicians
- Review and process physician orders and referrals in a timely manner
- Monitor compliance with Medicare conditions of participation and agency policies
- Assist with OASIS documentation review and quality assurance processes
- Support onboarding and orientation of new clinical field staff
Requirements
- Active RN or LPN license preferred; clinical background in home health required
- Minimum 2 years of home health clinical or coordination experience
- Strong knowledge of Medicare home health regulations and documentation standards
- Excellent organizational and communication skills
- Experience with home health EMR systems preferred
