Position Summary
The Clinical Coordinator III - RN is an integral member of the Neuroimmunology disease management team, responsible for phlebotomy (clinical and research), arranging infusions, and streamlining access to imaging. This role serves as the patient''s and family''s consistent point of contact by coordinating MRIs, infusions, and follow-up visits, and ensuring patients understand their plan of care.
Responsibilities
Patient Navigation & Education
- Navigates neuroimmunology patients through phlebotomy, infusion, and radiology services; coordinates multimodality scheduling, triage, and care across Neurology, Radiology, and Infusion services.
- Reviews labs and testing with providers, triages clinical questions, and ensures regular provider follow-up.
- Builds trust with patients and caregivers through active listening, empathy, and responsiveness; documents their feedback and relays it to the care team.
- Identifies and helps resolve financial, practical, social, and logistical barriers to timely care, engaging caregivers and other providers as needed.
- Serves as the liaison between patients, providers, front desk, and scheduling, ensuring smooth handoffs and continuity as patients move across care settings.
Patient Access & Appointment Coordination
- Coordinates scheduling across departments/specialties, including outreach, tracking, and follow-up.
- Triages incoming scheduling questions (including escalations from the call center/front desk) using clinical judgment.
- Gives patients clear instructions for appointments, testing, and procedures.
- Assists with insurance approvals and connecting patients to financial resources.
- Documents all encounters accurately in the EMR per regulatory and care standards.
- Responds to and resolves patient concerns promptly, escalating as needed.
People
- Helps onboard and train team members; coaches peers on performance.
- Stays current on care management and healthcare trends.
Strategy, Quality & Continuous Improvement
- Leads or supports performance-improvement projects related to navigation and care coordination; tracks KPIs and reports progress, including to senior leadership.
- Recommends and implements workflow/protocol improvements.
- Monitors quality metrics (e.g., time to first appointment) and reports on role effectiveness.
- Participates in quality improvement, evidence-based practice, and nursing research initiatives.
- Conducts chart review/outreach for quality reporting as needed.
- Upholds confidentiality, informed consent, and compliance standards (HIPAA, OSHA, etc.); completes required trainings.
- Performs other duties within scope of practice/training.
Minimum Qualifications
- Bachelor''s degree or equivalent in education and experience, plus four years or related experience.
- New York State RN license.
Preferred Qualifications
- Clinical nursing experience in neuroimmunology or related patient care.
- Experience with phlebotomy, including clinical and research blood draws.
- Strong communication, interpersonal, organizational, and critical-thinking skills.
